Dental Inlays vs Onlays: Which Option Is Right for You
When a tooth is damaged from decay or fracture but does not require a full crown, dental inlays and onlays offer an advanced, conservative solution. At Dental & TMJ Specialists of Greater DC in Bethesda, MD, our board-certified prosthodontists provide these highly customized restorations to restore strength, function, and esthetics while preserving as much healthy tooth structure as possible. For patients who value long-lasting results and minimally invasive care, inlays and onlays are often an ideal option.
Understanding Dental Inlays and Onlays
Dental inlays and dental onlays are custom-made restorations designed to repair teeth that have been weakened by decay, cracks, wear, or large fillings. They are fabricated outside of the mouth in a dental laboratory and then precisely bonded to the tooth using advanced adhesive techniques.
While similar, inlays and onlays differ in how much tooth they cover:
A dental inlay fits within the grooves of your tooth’s chewing surface. Inlays are recommended when the damage is limited to the center of the tooth and the outer structure remains strong.
A dental onlay extends over one or more cusps of the tooth. Often called a “partial crown,” an onlay restores more extensive damage while still preserving more natural structure than a full crown would. Onlays can also be used to improve tooth shape, color, and overall bite harmony.
Both inlays and onlays are custom-made from durable tooth-colored ceramics or gold. These materials are chosen for their strength, longevity, and ability to blend seamlessly with natural enamel.
Why Choose Dental Inlays and Onlays
Inlays and Onlays offer a number of benefits for patients who are looking for long-lasting, natural-looking restorations.
Compared to fillings, inlays and onlays are:
- Stronger and more durable for daily biting and chewing
- Less likely to shrink, wear, or stain over time
- Precisely fitted for improved long-term outcomes
Compared to crowns, inlays and onlays:
- Preserve more of your natural tooth
- Require less tooth reduction
- Offer a conservative alternative when a full crown is not yet necessary
For patients looking to fix a tooth that isn’t damaged enough for a full crown, inlays and onlays are an excellent, minimally invasive treatment choice.

What to Expect During the Process
Getting a dental inlay or onlay usually requires two visits. During the first appointment, your prosthodontist will carefully remove any decay or old restoration and prepare the tooth for a new inlay or onlay. An impression or digital scan is then taken and sent to a dental laboratory, where your custom restoration is made. You will receive a temporary restoration to protect the tooth while waiting for your permanent one to be completed.
At your second visit, our team will bond your inlay or onlay securely in place and ensure the bite is comfortable. Most patients experience minimal discomfort and can return to normal activities soon after the procedure.
